What You Need to Apply – You must apply in person and bring:
1. Proof of Identity
One of the following documents is required:
- U.S. birth certificate

2. Social Security Number
- Provide your SSN (card not required)
3. Proof of Arizona Residency
Two of the following documents is required:
- Must show your name and physical address

4. Name Change Documents
